Doubtless few will consider it anything more than a record of delirium…
Když je takto použito přídavné jméno (předpokládám, protože zde není čárka, jakou bych čekal v případě příslovce), použilo by se “a” v případě, kdyby byl význam “a few”?

Slovo doubtless chápu tady jako příslovce, a čárka není nutná:
Doubtless you have heard the news already.
“Doubtless” doesn't collocate well as an adjective with “few”. If it is used, the article would more likely be “the undoubting few” because it defines which “few” you mean. If you do use an adjective before “few”, you're right, it needs an article:
It’s really encouraging to see this generation of spec-wearing youngsters are facing less discrimination in the playground. (…) There will also be a reluctant few who might not want to wear their glasses, but by making sure they are comfortable and have had a say in style of frame, they are far more likely to wear them without any fuss.
“There will also be the reluctant few …” is also possible here.
