It's simpler than you think: if you Google images of „old maid“ – the stará panna, not the card game – you'll see that almost all the old maids conform to the stereptype of wearing a frilly cap. So if someone is described as „cappy“, it means that she tends to wear a cap.
Note that the word „cappy“ doesn't really exist. It's a nonce word, a one-off, an occasionalism, a hapax legomenon made up solely for the purpose of this witty spoonerism (happy and careless <> cappy and hairless).
